Garden Survey Suggestions

Hello everyone:

I need some help with developing a survey for community gardeners here in
Tacoma, Washington.  The focus of the survey is to determine gardener
"satisfaction" with their community gardening experience.  Has anyone used
a survey to measure "satisfaction" before?  Let me know.  Any suggestions
would be helpful.
