CG,

     I am part of a committee hoping to promote community gardeners in
     Nashville.  We are planning to conduct a survey of community
     gardens in Nashville in order to determine what their needs are and
     how our committee can help them have a more effective community
     garden.  We are also hoping to administer a survey to community
     groups who are hoping to start a garden in order to determine how
     we can help.  I am sure that these surveys exist in other cities. 
     Can someone send me one they feel works well?
