Someone has inquired about buying the rights to republish one of my columns and a picture for marketing purposes.  I'm looking for a standard form that I can use to grant them the rights and include a value for the contract.  Since the whole deal will be $1000 or less I really don't want to involve an attorney and was wondering if anyone has used a boiler plate or standard form for something like this and if you can point me towards it.
