I resorted to seeing a chiropractor.
The argument went something like this:
Chiropractor - it appears to be your neck. You will have to go easy on the 
lifting, and no digging for a while.
Me - I am sure it is my shoulder, I think I have adhesions in the tendon 
sheath. I have had this before.

By the third visit, now in agony, the Chiro decided to strap my shoulder 
back to see if it made any difference. It relieved the problem but not a 
lot. A week later, still in much pain, but in desperation to prepare a bed 
for some irises, I got out the tiller and started working quite close to a 
gum tree. Lots of roots of course. I was being soooo careful, but suddenly 
the tiller almost ripped my left arm off. I used several of Bills passwords, 
almost passed out from pain, my arm went numb and tingly, then the numbness 
passed off in a minute or so, and I went on tilling. The pain was gone 
completely.

It is not a 'cure' I would recommend to anyone, but I think it proved my 
point. I must have torn an adhesion in my shoulder and freed the tendon. Now 
my hips are playing up - I have so much weeding to do with all those new 
gardens :-)
