I found it!  Turns out I didn't recycle it after all.

It says
prepared by Dr. Lloyd Zurbrigg, Editor
April 1975.

The sources were
1) the ten year check lists (excluding the '29, which he didn't have at the time, but most/all of that info was also in the '39, I think) plus the annual R&I's through the 1974 pub. 2) 'catalogues of breeders and fanciers who have been interested in reblooming irises' - Lloyd Austin; Tell Muhlenstein in Provo Utah at the time; Fair Chance Farm, Beloit Kansas; Edith and Walter Buss, Vincennes, Indiana; Melrose, Stockton CA; Sid DuBose and Ben Hager; Chet Tompkins 'now of Canby, Oregon' [I didn't realize Tompkins was especially interested in rebloomers or that he wasn't always in OR] 3) Lloyd's own observations in Listowel, Stratford, and Kingston, southern Ontario, Canada; Bloomington, Indiana; and Radford Virginia.

Descriptions came from catalogs if he could find them if they weren't available in the R&Is (i.e., the '39 & '49 cumulative checklists)

The last 2 pages contain brief details about some/most/all of the breeders of rebloomers, plus 8 more irises not included in the main body of the checklist, no explanation for what they are doing stuck at the end. errata? last minute additions? a few look like he wasn't sure whether to include them or not (i.e. 'Blue Beard' - one of several different iris bearing the same name in CA.

Some tidbits I didn't know so thought were interesting - Gibson was in Porterville, CA.; there was a rebloom breeder in Chattanooga (practically a neighbor), Clint McDade; Frank Jones was in Trenton, NJ.
