I have had this same problem with small worms. After awhile I came to the
conclusion that these worms were the cleanup crew and the real problem was a
type of rot. It seems that the rot starts first and that the small worms are
eating the rot. This is adressed by adressing the causes of the rot. It does
seem to be associated with too much nitrogen, fertilizer and wet conditions.
The scape and treatment methods sesem to work best.
