Dana,
I expect most feed stores sell just about anything you might want to mix
into a custom fertilizer, from organic ingredients to chemical pelleted
fertilizers. If I were mixing a chemical fertilizer, I'd just buy bags
of potash, superphosphate, and nitrogen (and lime and gypsum for my
acidic clay) and mix them in whatever proportions I prefer. Actually, I
go heavy on the gypsum and potash (our soil is very potassium deficient),
don't tend to add chemical nitrogen at all (I do till in composted horse
manure between iris rows), and go light on superphosphate and lime.
I have also been known to brew up a batch of alfalfa/manure tea ...
incredibly stinky stuff, but the plants seem to enjoy it, and the deer
don't! Or I till in alfalfa meal when I redo beds or cultivate between
rows.
