AIS: article on 1993 awards changes?

I somehow came to believe that there was an AIS Bulletin article describing
the awards changes that took effect in 1993. I was going to photocopy it
and insert it into my Judge's Handbook. But scanning through back issues of
the Bulletin, I was unable to find it. The only thing close was a chart
illustrating the awards progression, but without the information on
eligibility periods, quotas, and so on. Was I imagining this? (Or does one
have to extract it from the printed board minutes?)
In a similar vein, when the Handbook was revised in 1984, we went to a
loose-leaf system so that it could be easily updated with replacement
pages. Were any such ever issued? If so, can they be obtained somehow?
I had a class on the new awards system given by Joe Ghio in Albuquerque in
1992, but I'd like to have an official written reference for the new system.
