I will take a stab at the crab apple question.  Do you fertilize it
regularly?  Is there a red cedar near by?  Have you looked at the leaves
that fall off.  Do they have small spots on them.  You might have
cedar-apple rust.  Check with a garden center for the appropiate type of
spray (fungicide) for the disease.
