Pumpkin Position

I know that the pumpkin should be at a 90 degree angle to the vine but
what do you do if there are leaves coming out at the same spot as the stem
of the pumpkin. It seems that everywhere I have a pumpkin there is a leaf
coming out at a 90 degree angle to the vine and the pumpkin is off to about
a 45 degree angle. Should I just cut off the leaf and start repositioning
the pumpkin and how close would I cut the leaf off to the vine? Would I
treat that cut with a anti-fungal paste? How big should the pumpkin be
before I do this? Or should I just leave well enough alone. Any help and
advice would be great!!
