over the top measuring

I have a question about over the top measuring.
Is the circumference measurement supposed to go from stem to blossom end or is it supposed to go parallel to the ground? These two ways of measurement are quite different in my case. My stem is quite high (which is why I've been suffering from stem splits) and the blossom very low and will give me one measurement. If I go parallel to the ground the measurement is quite a bit larger. Which method is most accurate?
