Harry's suggestions (

   Acer saccharum 'Monumentale' (same as 'Temple's Upright')
   Koelreuteria paniculata 'Fastigiata' (would get tip damage in really
                                         cold weather)

) are both suitable for zone 4/5.  The only one that won't do
so well would be the fastigiate dogwood.  

Other trees that have been mentioned that would do well in zone 4/5
include:

   Acer platanoides 'Crimson Sentry'
   Acer rubrum 'Scanlon'
   Acer saccharum 'Goldspire'
   Acer saccharum 'Newton Sentry'
   Alnus glutinosa 'Pyramidalis'
   Betula platyphylla 'Dakota Pinnacle' (marginal in zone 4)
   Fagus sylvatica 'Dawyck Gold'
   Fagus sylvatica 'Dawyck Purple'
   Fagus sylvatica 'Dawyck'
   Liriodendron tulipifera 'Fastigiatum'
   Quercus robur fastigiata
