The milky sap is a sign that the lettuce is about to bolt to seed. If the
leaves still taste good, you should harvest the entire plant immediately. If
the leaves are bitter, well, there's always next year. Incidentally, the
milky sap is where the genus name, _Lactuca_, is derived.
