Broccoli going to flower

I planted 6 broccoli transplants, l per square, on April 30.  They were
about 3 inches tall.  I now have lovely plants of various heights (12-16
inches tall) but they are nothing but yellow flowers.  It started to look
like broccoli a few weeks ago but never formed a head.  What did I do
wrong?

BTW, I'm in Columbus, Ohio where the weather has been rather cold and
quite rainy.
