I have noticed these same "holes" in my bearded irises. I believe that you
are correct in assuming that these are what's left after the older roots
die. I've even created a few new "holes" by pulling out the old roots
while cleaning and dividing my irises. If you had a pest doing this, you
would undoubtedly have some serious rhizome rot on your hands by now.:)
