Here is the Notice that Michelle Snyder sent out a while back to the RVPs
(thanks to Judy Keisling for locating it!)

AIS Insurance b Procedural Change
There has been a slight change in the procedure for requesting AIS
certificates of insurance.
When requesting a certificate of insurance, the requester will be asked to
verify the address of the AIS. It is my home address of 7855 Ellenbogen St.,
Sunland, CA. 91040.
