  that is two good questions !! when I am training vines I go out one night 
and remove the tendrils and turtiaries when they are very small at this time I 
make my small trench that my vine will lay in. I hold the vine in position 
with Shish kabob sticks in a X shape to hold then where I need them to be. the 
next night I go out and bury the vine where it was trimmed at this point the air 
has dried up the wounds and the vine is ready to be buried and I work on the 
next section along the vine that is ready to be trimmed and follow this 
procedure till I am done !! 
   Now where to trim the leaves I think you are refering to the runner at the 
leaf. when you cut a runner in the middle it will be Hollow if you keep 
cutting back a little at a time you will make it to the LEAF AXIL at this point it 
will become solid. try and cut the runner 1/2 inch after the leaf axil on the 
last leaf you want to keep if it is nice and developed or just gently pinch 
off the leaf buds in the tip of the runner if it is small remembering that the 
runner will still elongate till the last leaf on the remaining runner has its 
proper placement !! sounds difficult but it becomes second nature after a few 
weeks !!!
