This is a result of damping off, caused by one of four organisims. 
pythium, phytopthera, botrytis or or rhizoctonia.  The most assured way
to control it is to drench with a broad spectrum fungicide such as
Banrot.  This is not the most effective agent for all, but is the most
emcompassing.  Most of these critters are spread by water, so avoid
splashing when watering.  Actually, water from the bottom and the
surface will stay dry, helping to avoid setting the stage.
Many organisms need moisture for a certain period of time to sporulate,
so keeping good air circualtion will dry the excess before spores can
get going.  Mist beds overcome this by rinsing the spores away before
they can sporulate.
