Duncan,
I do both. I grow a lettuce mix in a container as a mesclun (crowded) and
harvest the leaves by cutting them about 1" above the soil. They grow back
in about 3 weeks.
In my garden, I harvest whole heads of butterhead lettuce (my favourite) by
pulling up the whole plant when it reaches a good size.
