Review copies


Hi folks,
I'd like to visit reviewing books but from a different perspective.

Is there a protocol for getting review copies?
Do we just ask the publisher to send us a copy, and they do it? (My experience so far has been yes, but is this the norm?)
If a book shows up, unsolicited but with the request that you write a review, is there any obligation? (I assume not but I am also very leering of "assuming" anything in this business.) I have a few of these sitting on my desk.
Who pays for the copy- the author? the publisher? I've received books from those two "entitiies" as well as from agents. Is the assumption that the review ( not that I'm egotistical, mind you) will generate book sales far beyond the cost of the "free" book?
When you receive a review copy and you actually write a review to whom to do you send it? I like to publish it in my column and send a copy to the author. Is this the norm?
Is there a time frame from receiving a book to publishing the review?
What are your thoughts?
