Our local clinic recommends tetanus vaccination routinely every 10 yrs, 
but they also recommend (as apparently does your clinic) that if you 
haven't had a booster within the 5 yrs prior to a puncture wound, you 
should come in immediately for a booster after such an injury.  This is 
especially essential for those of us working around horse manure (in 
which tetanus apparently thrives) on a regular basis.
