I wonder if part of your problem may be that you fertilize all the time, whether with Miracle Gro or some other product that had lots of nitrogen in it.  Can't say definitely that that is the cause of falling, but iris don't need a lot of fertilizer, after all, except the rebloomers.  I have not used any fertilizer at all for years, but they keep giving me spectacular bloom every year, and the plants are mighty healthy, too.  I even dilute the good Iowa dirt with sand when I plant a new bed.   Just a possibility worth considering.
