I have seen this in my garden also.  More often in clumps that need  divided. 
 It seems to correct itself as the season progresses, which  suggests to me 
either bacteria or cold damage.  This year I had it in a  clump of Victoria 
Falls and Jurassic Park.  Both of these can rebloom so I  wonder if the 
bloomstalk could be damaged while it is still in the fan?  I  have seen it in previous 
years in clumps that had some slug damage, but this  year I didn't have any 
slug damage at all.  The problem with the frost  damage theory is that some of 
my early TB's had obvious frost damage including  dried buds yet didn't have 
this anomaly.
