Dear nan,
I keep a looseleaf binder with all my propagation projects in it, one 
per page, in alphabetical order. Plant provenance, plant associates, 
type of propagationn material, material source, number of seeds or 
whatever used, any special treatments, etc. are all noted and then I 
update the page when I pot up or plant out. I can't say I keep track of 
everything you've mentioned, but I could I guess.
