 OK let me give it a stab.... The leaf Damage is defiantly from the aphids. 
Try using Immunox Plus it will kill the Aphids and keep several diseases at bay 
like Powdery mildew. You can get it at your local garden shop and you use two 
ounces per gallon in a compression sprayer about every two weeks GREAT STUFF 
!!! 
   Now for the Runners I am having somewhat the same problem if I cover the 
vine with soil it will be disintegrated in four days. You may have soggy soil 
that is building up a disease but for now on that plant looks like the stump is 
Done. I am still averaging 14 to 20 lbs a day on plants that are a mess. this 
year I am going to have a tissue sample done to figure out what the disease 
is and then decide if I can fix it. 
   if the plant goes down completely try laying clear plastic on the soil and 
solarize it. depending on how many more days of heat you have you might be 
able to kill any diseases before they get a strong hold on your patch like in my 
patch... I will post to the list when I find out what I have in my soil. take
